1. It is vital to keep your greyhound on leash whenever it is outdoors, unless it is in a fenced-in area. NEVER trust" a greyhound not to run away.
2. You must keep a greyhound safety collar bearing identification on your greyhound at all times.
3. You must never use a retractable leash on your greyhound.
4. If your greyhound should become lost, you are required to notify the Greyhound Adoption Center.
5. If you should ever decide you cannot keep or do not want your greyhound, you must notify the Greyhound Adoption Center before taking any action. You may not give your greyhound to anyone else without the Greyhound Adoption Center's consent and you must agree never to take it to a "pound" or other humane shelter.
6. You must agree to keep your greyhound as a pet and never attempt to use it for pari-mutuel racing or laboratory research.
7. The Greyhound Adoption Center will have performed the following medical procedures for your greyhound: DHLPP, Rabies, Bordatella and Corona vaccinations; spay or neuter, teeth cleaning, polishing and any extractions that may have been necessary; worming; a blood panel that includes Thyroid and Heartworm tests. Your responsibilities will include a fecal exam within 30 days of adoption, and maintaining regular vaccinations and veterinary care whenever they are required.
8. Your greyhound's racing career is over and can never be reactivated.
9. The Greyhound Adoption Center is a non-profit, volunteer-run organization funded primarily by donations. There are many expenses associated with the rescue and placement of each greyhound. A donation of $80.00 is required at the time of adoption, as well as an additional fee of $160.00 which partially helps to cover the costs of the medical procedures described in #7 above. The adoption donation and fee total $240.00. Your greyhound will be provided with a greyhound safety collar at no charge. A matching leash is available at a nominal fee. The adoption donation and additional fee for the medical procedures are non-refundable.
This application, if approved, will become part of the greyhound adoption agreement.
I understand that in order to complete the processing of this application, a visit to my home will be scheduled by a local representative of the adoption program. By submitting this application, I agree to such a scheduled visit. I certify that all information provided by me on this application is true and correct.
I have read and agree to abide by the requirements set forth. If I qualify and receive a greyhound from the Greyhound Adoption Center, I will accept full responsibility for the greyhound.
